All 154th LSO soldiers requesting orders (TDY, AT, ADT, or ADSW) should use this page to submit their request electronically to the 154th HQ. Please complete the form as fully as possible and be sure to include your team information..

ATTENTION: If you are submitting a request for any type of Annual Training, your order must cover all time while in a travel status. For example, if you must begin traveling at 1600 on Friday in order to arrive for a separation board on a Saturday, then use Friday at 1600 as your report time. If you anticipate the board will be finished on Saturday, and that you will also complete return travel on Saturday, your number of duty days would be "two," rather than "one." In this instance, many people have mistakenly used the time the board begins on Saturday as their report time and "one" as the number of duty days.
